The Lab at LeConte Medical Center is a rewarding place to work. Position Summary:

Responsible for coordinating the Stroke Program at Parkwest Medical Center by working in conjunction with the Stroke Medical Director, Stroke Nurse Practitioner, Regulatory, Quality, and Hospital Administration to maintain Primary Stroke Certification.

Responsibilities

  • Manages and coordinates stroke patient care during and post hospitalization.
    • Lead development of care plans, protocols, order sets, and other guidelines in the management of stroke including TIAs, ischemic stroke, and hemorrhagic stroke.
  • Facilitates Stroke Community Events including coordinating “screening stations”, arranging for guest speakers, etc.
  • Monitors and provides on-going staff educational opportunities for all levels and departments interacting with the Stroke population.
    • Oversees compliance with stroke education requirements for all hospital nurses in conjunction with the Stroke Manager and ED Stroke Manager.
    • Ensures education and compliance of EMS
  • Collects and analyzes on-going data regarding outcomes of the Stroke Program. Submits required data to the Joint Commission and other regulatory agencies as requested.
    • Examples: patient satisfaction, discharge phone calls, Joint Commission required measures, Get with the Guidelines measures,  clinical outcomes, present findings to the FAST Committee and other hospital committees as required.)
    • Maintains an easily-reviewable database with all required metrics.
    • Ensures compliance with Joint Commission standards on all metrics and identifies areas in which improvement can and should be made.
      • Leads the recertification process with identification of areas of non-compliance and methods in which this could be corrected.
      • Identifies and designs quality improvement projects dedicated to improving patient care.
  • Performs and evaluates effectiveness of patient teaching.
    • Examples: development of patient teaching materials, seeks feedback from patients/physicians/staff to determine if teaching is effective.
  • Maintains clinical knowledge and skills for the stroke patient population and acts as a consultant to nursing staff.
  • Acts as a resource to the hospital for care of the stroke patient regardless of admitting diagnosis of the patient.
  • Participates in stroke directed research including acquisition of data in addition to TJC required data.
  • Maintain established stroke care policies and procedures, objectives, quality assurance program, and safety standards.
  • Works cooperatively and effectively with all administrative, nursing, medical and paramedical personnel.
  • Coordinates, directs and motivates personnel.
  • Demonstrates initiative, insight and judgment in making decisions.
  • Provides leadership in solving problems.
  • Participates in meetings and committees as required.
  • Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
  • Completes continuing education on cerebrovascular disease, including acute stroke care to meet the requirements for participating on the Core Stroke Team.
  • Other duties as assigned and special projects as identified by nursing administration.

Qualifications

Minimum Education:           

None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.

 

Minimum Experience:         

Five (5) years’ experience in nursing. 

 

Licensure Requirement:      

Must have current and maintained Tennessee RN nursing license.
