Position Summary:

This position provides a variety of functions that support the operations of the Health Information Management Department. Specific position may not perform every function listed below. This position is responsible for preparing documents for imaging in the MedHost optical imaging system. This Technician prepares the documents for scanning, scans the various documents and performs routine maintenance on the scanning equipment. This position is also responsible for the indexing of the documents to the appropriate medical record/account and to the appropriate document type within the record. Lack of proper indexing may result in the loss of vital data. They also monitor all scanned and indexed documents for quality. This individual analyzes the records online for the assignment of deficiencies, according to the Medical Staff Rules and Regulations. Efficiency and good organizational skills are required. Attention to detail is necessary, in that the lack of detail when performing these functions may result in the loss of vital information. This Technician is responsible for recording batch information and also for preparing scanned records to be removed to off-site storage.

 

Additional tasks include, answering the telephone, assisting department customers, and distributing copies of dictation. Processes all written and verbal requests for medical record information from various individuals/agencies, ensuring proper authorization. Conducts other tasks as requested.

Responsibilities

  • Prepares documents for scanning. Removes staples/paper clips, repairs torn pages, records account number if it is not present, photocopies documents not suitable for scanning marking document with an “original document poor quality” stamp.
  • Separates daily discharged records by medical record number. Sorts loose document by number of sides/number of sheets. Inserts batch covers sheets and if necessary document separator sheets. Records batch information on cover sheet. Places the prepared batch into the ready to be scanned area.
  • Ensures proper settings on scanner and software controls, feeds documents through scanner, checks for quality of scanned documents and prepares documents for post-scan storage.
  • Performs required maintenance on scanning equipment including cleaning the image guides and rollers and calibrating the scanner according to the maintenance schedule (refer to the scanner manual for additional details on scanner maintenance).
  • Enters the account number and document type for each document scanned, as needed. Deletes blank documents and identifies documents for re-scan.
  • Analyzes the on line scanned documents for completeness. Assigns physician deficiencies according to the Medical Staff Rules and Regulations. Re-analyzes records upon physician completion.
  • Monitors all aspects of the scanned documents for quality, ensuring legibility and completeness of images.
  • Assists in answering incoming telephone calls and routes to the appropriate individual section. Obtains phone messages and either provides caller with requested information or communicates written request to the appropriate individual.
  • Assists visitors, physicians, and hospital personnel who enter the department as needed.
  • Distributes mail, orders supplies, and monitor various department equipment obtaining service when needed. Performs other duties as assigned.
  • Demonstrates ability to meet or exceed departmental quality and quantity standards.
  • Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.

Qualifications

Minimum Education:          

None specified; will accept any combination of formal education and/or prior work experience sufficient to demonstrate possession of the knowledge, skill and ability needed to perform the essential tasks of the job, typically such as would be equivalent to a high school diploma or GED. Preference may be given to individuals possessing a HS diploma or GED.

 

Minimum Experience:         

One (1) year experience in medical records department preferred.
