Clinical Coordinator, Surgical Associates of East Tennessee

Full Time, 80 Hours Per Pay Period, Day Shift

Position Summary: 

To assist in seamless patient flow from initial contact to end of care.

Responsibilities

  • Participates in multidisciplinary teams to improve care of designated patient population.
  • Meets with new patients / families to discuss plan of action at initial evaluation and serves as liaison between specialist, MD, patient/family and primary care physicians.
  • Properly schedules patient appointments, and assists patients with preparing for appointments, when necessary.
  • Develops and distributes educational materials designed to foster the patient’s and family’s understanding of the plan of care.
  • Recognizes the appropriate appointment type and timing based on referral information and records.
  • Demonstrates expertise in the referral process and use of community resources.
  • Gathers patient information from office charts for use in database.
  • Understands and enters clinical data into patient information database.
  • Monitors the achievement of identified outcomes.
  • Assess patient’s general condition and takes vital signs and weight.
  • Conducts follow-up phone calls to patients for assessment of clinical status.
  • Makes follow-up phone calls to primary care MD office to advise of the status of the patient.
  • Participates in community outreach as it relates to the clinic.
  • Appropriately tracks diagnostic studies (mammograms, MRI, ultrasounds) to expedite patient care.
  • Identifies information needed and obtains appropriate imaging, pathology and medical records reports prior to patient appointments.
  • Assists provider with examinations, diagnostic procedures, and treatments.
  • Demonstrates ability to properly explain various testing procedures that are specific to each patient.
  • Distributes patient satisfaction surveys.
  • Provide basic patient care as ordered.
  • Maintains confidentiality in matters regarding the practice, its patients, and employees.
  • Supports the patient and family through the process.
  • Local travel required.
  • Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives and participates in quality improvement initiatives as requested.
  • Performs other duties as assigned.

Qualifications

Minimum Education:          

None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority if applicable.

 

Minimum Experience:         

Two (2) years’ experience required as an LPN or certified Medical Assistant from a recognized certifying organization.

 

Licensure Requirement:      

Must be certified as a Medical Assistant from a recognized certifying organization or must be licensed as an LPN in the state of Tennessee. American Heart Association BLS required. Employee must have a valid Tennessee driver’s license Class D and state mandated minimum insurance coverage.  Driving record must meet Covenant Health minimum standards at the date of hire and throughout employment tenure.
