Clinical Manager, HomeCare

Full Time, 80 Hours Per Pay Period, Variable Shifts

Manager for Hospice - participate in administrative call rotation.

Position Summary:

The Clinical Manager coordinates the provision of resources to staff so patients may receive safe, holistic care. Sets standards of care and practice and evaluates the services provided for patient population served. Represents patients, staff and administration in an effort to achieve continuous improvement in the quality of services. The Clinical Manager reports to the Home Health/Hospice Director.

Responsibilities

  • Provides for ongoing management and supervision of designated staff to include on-site evaluation, documentation, deficiency reviews, personnel action plan development and clinical decision making.
  • Oversight of staff compliance and monitoring of the productivity standard.
  • Receives orders from MD as needed after field staff has communicated changes in patient assessment to MD and communicates additional orders to staff as appropriate. Co-signs MD orders as needed.
  • Responsible for analyzing and ensuring proper staff utilization across HomeCare service area.
  • Participates in the fiscal planning of the department.
  • Works in conjunction with Quality Department to lead branch Process Improvement efforts, action planning, follow through and communication.
  • Provides ongoing customer service to include service recovery related to patients, physicians, vendors etc….
  • Ensures that all complaints are addressed in a proactive manner and that Process Improvement is integrated into the work culture of the staff in areas of responsibility.
  • Approves and schedules CTO time.
  • Analyzes and identifies excess staff, or need for additional staffing, according to approved staffing ratios and submits to HomeCare/Hospice Director. Actively participates in the hiring process.
  • Assures accuracy, completeness, and timeliness of all documentation. May serve as member of the Audit Team.
  • Directs the staff to ensure OASIS data and forms are timely, complete and accurate.
  • Notifies leadership/billing office of failure to obtain orders for services provided.
  • Maintains current knowledge of compliance guidelines and assures teams’ activities meet standards.
  • Maintains current knowledge of agency policy/procedures and educates staff regarding policy and procedure and revisions.
  • Responsible for selection, job specific training, performance assessment, discipline, and termination of all assigned staff.
  • Schedules staff meetings regularly with all staff members to provide opportunity for problem solving and communication of information.  Identifies opportunities for improvement in processes and communicates to HomeCare/Hospice Director.
  • Coordinates staff education needs with the Education Department.
  • Attends twelve (12) hours of continuing education programs, seminars, and personal professional in-service per year.
  • Actively participates on agency committees, task forces and teams when requested.
  • Assures needs of team are met and reports needs to HomeCare/Hospice Director.
  • Serves as manager designee in the absence of the HomeCare/Hospice Director.
  • Maintains professional growth and development through seminars, workshops and professional affiliations to keep abreast of latest trends in field of expertise.
  • Attends meetings as required.
  • Works with HomeCare/Hospice Director to insure all new employees complete required orientation and training.
  • Depending on patient census, may be required to perform patient visits in area of clinical licensure. Must stay current with patient care competency requirements.
  •  Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives and participates in quality improvement initiatives as requested.
  • Performs other duties as assigned.

Qualifications

Minimum Education:           

None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority. Possession of a bachelor's degree in nursing or a related field preferred.

 

Minimum Experience:          

Three (3) years of experience in supervisory position with demonstrated successful leadership skills.  Excellent communication ability and computer skills. Knowledge of home health and hospice state licensure requirements.  Knowledge of CHAP requirements and regulations. Hospice exerience strongly preferred. 

 

Licensure Requirement:      

Must have and maintain state registered nurse license. Employee must have a valid Tennessee driver’s license Class D and state mandated minimum insurance coverage. Driving record must meet Covenant Health minimum standards at the date of hire and throughout employment tenure. OASIS certification required for HomeCare (must be obtained within 90 days of employment).
