Position Summary:

As a Quality Improvement leader for Covenant HomeCare and Hospice and Home Medical Equipment, works with the Leadership Team to design and implement the quality improvement processes that support the delivery of patient care. Reports to the Director of HomeCare and Hospice.

Responsibilities

  • Participates in the development of plans, education and process review activities designed to insure the delivery of quality care and performance improvement.
  • Participates with the clinical leadership team in implementing quality management tools and processes. 
  • Prepares for and is involved in the completion of facility regulatory and licensure surveys.
  • Coordinates the data collection activities and assists the leadership team in the communication of the outcomes of such activities to appropriate parties. 
  • Assists with the development, coordination, and implementation of operational policies/procedures as appropriate.
  • Orients and trains staff in applicable areas, including performance improvement activities, CovLearn module assignment, etc.
  • Supports field staff by providing education in the field and in-house, including OASIS training
  • Where appropriate, will assist in resolving customer complaints and grievances by providing appropriate data and input.
  • Involved in Preceptor Program, monitoring for completion, success, and continual improvement.
  • Serves as liaison for HomeCare for the system Education Team.
  • Serves as liaison for HomeCare for the system Policy and Procedure Team.
  • Assists in software related upgrades.
  • Coordinates outside education requests as well as CPR and other common educational needs.
  • Looks for opportunities for process improvement and follows the PDCA cycle to make improvements to include education to staff to develop and instill a quality culture throughout the organization.
  • Reports audit results to departments, leadership, and in team meeting settings as appropriate.
  • Completes applicable monthly reports, including but not limited to: various scorecards, SH, PRC, Hospice, wound reportables, etc.
  • Assists in flu clinics as needed.
  • Fulfills various infection control related duties, including but not limited to: Pandemic/Disaster related contact, monthly infection control tracking, etc.
  • Manages compliance as applicable including but not limited to the following tasks: tracking tools, HIPAA, regulatory changes, etc.
  • Reviews charts for purposes of prebill, ADR, appeals, RAC.

Qualifications

Minimum Education:                      

None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.

 

Minimum Experience:                     

At least five (5) years of clinical experience, with at least two (2) years of HomeCare/Hospice experience.   Prefer experience in quality improvement or risk management field.

 

Licensure Requirement:                  

Current licensure as a Registered Nurse in the State of Tennessee.
