Clinical Social Worker, Quality Management

Full time, 80 Hours per pay period, Day shift

Some travel to clinics required- multiple counties where CMG clinics are located.

Specialties include cardiology, cardiothoracic surgery, cardiovascular surgery, endocrinology, gastroenterology, general surgery, infectious disease, neurology, neurosurgery, obstetrics and gynecology, occupational medicine, orthopedic surgery, physical medicine and rehabilitation, primary care, pulmonology, reproductive medicine, rheumatology, sleep medicine and urology.

 

Position Summary:

Facilitate patient continuity of care as appropriate through coordination/implementation of services to ensure a smooth continuum of care. Advocate and liaison for patients and families, helps formulate resources for financial and community needs.

Responsibilities

  • Liaison for patient/family, treatment team and external providers.
  • Schedules/facilitates patient and family conferences as indicated to discuss plan of care.
  • Serves as patient/family advocate.
  • Performs complete psychosocial assessments as indicated.
  • Provides on-going patient assessments according to clinical and ethical standards.
  • Assures patient is being served at the most appropriate level of care.
  • Promotes efficient coordinated care with multidisciplinary treatment team including external providers.
  • Facilitates/coordinates appropriate patient level of care through coordination/implementation of services to ensure a smooth continuum of care.
  • Utilizes community resources and support services, providing referrals as needed.
  • Utilizes cost management strategies by reducing readmissions, inappropriate ED utilization, coordinating financial resources, and identifying alternate resources for funding as needed.
  • Verifies completeness of patient and family education/training.
  • Maintains knowledge of regulatory standards and Quality Measures for all participating payers.
  • Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
  • Perform other related duties as assigned or requested.

Qualifications

Minimum Education :          

Masters’ degree from CSWE accredited graduate social work program.

 

Minimum Experience:         

Two (2) years of experience.

 

Licensure Requirement:      

Licensed Clinical Social Worker (LCSW) required.
