Position Summary:

The Lung Navigator provides clinical support, treatment access, education, and coordination of patient-centered care to benefit the patient, family and multidisciplinary physicians while helping achieve optimal outcomes for cancer patients and their families.  This position will serve as the navigator for the Lung Nodule Program (LNP) and the Epilepsy Monitoring Program (EMP).

Responsibilities

  • Serves as the essential link between patients and other care providers to provide resources and assistance with accessing clinical and supportive care services offered within the healthcare system and the community.
  • Reviews potential candidates for the lung nodule program, screens candidates according to criteria established by Vascular and Pulmonology and prepares list for review by multidisciplinary providers. For patients selected, will contact the patient’s provider to suggest a pathway into the Lung Nodule Program.
  • Provides CMS regulatory reporting for Low Dose CT Lung Screening Program.
  • Coordinates with the Neurologist/Epileptologist office, patients, Neurodiagnostics, nursing unit and Registration for scheduling inpatient visits to the EMU.
  • Coordinates follow-up appointments with Pulmonology Intervention and/or Thoracic Surgery for the LNC.
  • Coordinates follow-up diagnostic tests and procedures for the EMP as directed by the EMP providers.
  • Advocates on behalf of the patient.
  • Oversees and participates in all multidisciplinary conferences.
  • Follows patients throughout the continuum of care for a time period defined by the respective programs as outlined.
  • Aids in coordination of care between various healthcare providers and facilities, as needed.

Qualifications

Minimum Education:  None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.

 

Minimum Experience:Minimum of 3 years of experience in a hospital or physician office practice as a RN or in an advanced Imaging modality.

 

Licensure Requirements:Valid Tennessee RN license or certification in an advanced Imaging modality (CT, MRI, Ultrasound, PET, Nuc Med, Radiation Oncology, or IVR.)
