Speech Pathologist, Home Health 

Full-time, 72 hours per pay period, Day Shift

Position Summary: The Home Care Speech/Language Pathologist will identify functional speech defects and provide treatment to patients with communicative or swallowing disorders. Provides instruction and supervision of clinical fellow interns/externs and other clinical staff. Provide scheduling information and make scheduling decisions. Facilitate program development and enhance guest relations within the home care setting. The Speech Pathologist reports to the assigned Clinical Supervisor.

  • Conducts initial and ongoing assessments to identify the patient’s level of functioning, ability to communicate, home environment and equipment needs; including OASIS assessments at appropriate time points.
  • Develops the plan of care in consultation with the physician and other interdisciplinary care team members and revises as appropriate.
  • Performs all speech therapy documentation in medical records for assigned patients; including clinical and progress notes.
  • Identifies speech deficits, utilizes specialized therapeutic technique and/or equipment, and recommends communication devices/aids as needed.
  • Provides speech‑language pathology or audiology services in accordance with the interdisciplinary plan of care; including discharge planning.
  • Educates and consults with the family and other agency personnel.
  • Observes and supervises CFY s, students and externs.
  • Coordinates patient care and discharge planning with other health care professionals including MD, nurses, and other therapists.
  • Provides professional guidance to other staff.
  • Consults with Clinical Supervisor regarding departmental issues and concerns.
  • Recommends appropriate inter‑agency and intra‑agency referrals.
  • Sets goals based on needs of hospice patient and caregiver.
  • Tests hearing and recommends mechanisms which focus on such interventions as alternative methods of communication, speech, swallowing exercises to help with nutrition.
  • Supervises home health aide in procedures directly related to the speech-language services.
  • Communicates information to appropriate team members.
  • Local travel required.
  • Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
  • Performs other duties as assigned.


Minimum Education: 

Master's degree from an accredited Speech Pathology Program


Minimum Experience: 

Two (2) years’ experience with at least one (1) year experience in the specialty area. Professional organization membership preferred


Licensure Requirements: 

Must have and maintain Tennessee Speech Language Pathologist licensure and ASHA certification. CPR required. Employee must have a valid Tennessee driver’s license and state mandated minimum insurance coverage. Driving record must meet Covenant Health minimum standards at the date of hire and throughout employment tenure. Current CPR required.
