Position Summary:

Provides professional social work services to meet the needs of the patients in the hospital. Provides counseling, crisis intervention, and discharge planning to patients and families in the hospital. Assists patients and their families in seeking help with emotional, social, psychological and financial problems. Incorporates documentation of these services into the active medical record. Coordinates these services with other departments to insure continuity of care.

Responsibilities

  • Facilitate discharges of hospital patients to nursing homes, or home health services. Complete all necessary paperwork and provide or release information via the portal to receiving facilities.
  • Assist with arranging and/or obtaining Durable Medical Equipment (DME) as necessary to facilitate discharge.
  • Participates in Interdisciplinary Discharge Planning Rounds twice weekly.
  • Coordinates patient care with nursing staff, physicians, case manager, utilization review coordinator, home health coordinator by keeping them informed of progress with patients via verbal communication and chart notes (hand written or computer) in the patients medical record.
  • Provide Crisis intervention in the Emergency Department or wherever needed both situational and psychosocial. Refer to mobile crisis when appropriate. Address Lakeshore transfer concerns.
  • Make appropriate social service referrals to community resources, address situations with inadequate social support.
  • Assess psychosocial needs of the patients and develop a plan to address the needs identified.
  • Facilitate adoption or agency referrals in cases of abandoned baby or other infant/child cases.
  • Assist in the execution of advanced directives.
  • Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
  • Promotes good interpersonal and interdepartmental relationships by working with all members of the health care team to ensure maximum physical and emotional care of the hospital patients.
  • Provides on-going clinical social work to patients and families, monitoring daily psycho-social, emotional, and physical adjustments.
  • Maintains monthly statistical report on status of caseload.
  • Attends meetings and serves on committees as required.
  • Provide employee counseling as necessary.
  • Perform other related duties as assigned or requested.

Qualifications

Minimum Education: Bachelor’s Degree from CSWE accredited baccalaureate social work program.

 

Minimum Experience: Two (2) years of experience.

 

Licensure Requirement: None required.
