    Full-Time, 80 Hours per pay period, Variable shifts

    Position Summary:  The Emergency Department Medication Reconciliation/Discharge Planner is responsible for integrating evidenced based clinical practice into the patient care setting, promoting patient care continuity with care transitions. The position has accountability for medication reconciliation and providing comprehensive care coordination with the supervision of a Registered Nurse Case Manager to plan, implement, monitor and evaluate the outcomes of care for the designated patient population. Activities and interventions include maintaining patient’s privacy, confidentiality, safety, advocacy, adherence to ethical, legal, and accreditation/regulatory standards.


    Minimum Education :  None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority. 


    Minimum Experience:  Three (3) years of acute care nursing experience; a minimum of two (2) years of experience in area of assigned responsibility.  Experience in Case Management preferred but not required.


    Licensure Requirement:  Current Tennessee LPN license
