Transitional Care Liaison
The Transitional Care Liaison is responsible for the facilitation of transitional care activities from the initial hospital admission to the hospital discharge process, including the coordination and the hand-off procedures from the inpatient setting to the post-acute, outpatient setting. This is accomplished by focusing first and foremost on utilizing proprietary EMR in the identification and communication of the targeted patient population for physician teams. Additional activities may include, but not limited to, access across different EMR’s of patient information, creation of a readmission risk assessment, coordination of discharge planning, development of post‐acute care coordination plan, follow‐up on patient progress relative to the post‐acute plans, etc.
Bachelors degree required in Nursing or related field
Four (4) years of acute care nursing experience; a minimum of three (3) years of experience in area of assigned responsibility. Must have experience working with Physicians regarding transitions of care. Quality Improvement/Quality Assurance/Case Management background preferred.
Current Tennessee RN License. Current certification in Case Management, CCM, ACM or CPHQ required.
