Position Summary: Functions as a liaison between referral source and home health agency to facilitate coordination of care; collects needed information to arrange home care and associated services and educates patients/families, physicians, and physician’s office and other appropriate staff about home care services. The HomeCare Coordinator reports to the Director of Support Services.
Minimum Experience: Minimum of five (5) years of nursing experience preferably in home health. Thorough knowledge of services provided by home health; knowledge of reimbursement methods such as Medicare and private pay. Experience in case management or discharge planning desirable.
Licensure Requirement: Must have current RN license in the State of Tennessee. Employee must have a valid Tennessee driver’s license Class D and state mandated minimum insurance coverage. Driving record must meet Covenant Health minimum standards at the date of hire and throughout employment tenure. CPR required.
